Post cataract surgery endophthalmitis

Acute post surgical endophthalmitis is always a very serious event which
needs accurate monitoring and urgent intervention when signs and symptoms
get worse. Sometimes the surgeon is faced with very advanced stages of
endophthalmitis which have seriously compromised the eye ball. In these
cases, the first goal is to save the eye ball which would otherwise be
destined to phthisis and secondly, to achieve the best functional results.
The video shows some cases of advanced endophthalmitis where we use early
injection of heavy liquid to guarantee higher safety to the vitrectomy,
which is performed in a difficult condition. The fixation of the posterior
retinal surface with HL allows us to perform the manoeuvres in a fast
and safe way both on the anterior vitreous and the retinal surface thanks
to the “working under PFCL” techniques. In this situation,
the heavy liquid bubble is a third hand as an atraumatic hydrodinamic
manipulator.
