Surgical management of endogenous fungal endophthalmitis

Purpose:The purpose of this paper is to present our results in the surgical management of endogenous fungal endophthalmitis.

Methods: Six patients with clinical picture of endogenous fungal endophthalmitis were treated with pars plana vitrectomy combined with Amphothericin B intravitreous injection. Systemic antifungal therapy was additionally applied in the most severe cases.

Results :The diagnosis of Candida endogenous endophthalmitis in five patients and Aspergillus endogenous endophthalmitis in one patient was made by obtaining the vitreous material during the surgery for microscopic evaluation and culture. Good anatomical and functional results were obtained in four of six treated patients.
Conclusions:
1. Pars plana vitrectomy in the management of endogenous fungal endophthalmitis makes it possible to collect material for culture, removal of viable organisms and inflammatory products, and drug administration.
2. In the most severe cases we recommend silicone oil tamponade during first procedure to prevent retinal detachment and inflammatory process recurring.
