phaco tip advantages

New phaco systems, like the "minimal stress" system, allow a
decrease in the ultrasound duration and power, reduce the risk of incision
burn, permitting to remove the infusion around the phaco tip, which can
lead to a reduction of the corneal incision. We can discuss on the advantages
of performing a phaco through a 1.3 scleral incision, since we must today
increase this insicion when inserting a 6 or 6.5 mm IOL. In combined surgery,
however, minimal stress system presents two major advantages:
- standard phaco before vitrectomy: thanks to a phaco through a 1.3 mm
corneal incision the following vitrectomy is performed without problems
of corneal leakage. At the end of the procedure, the corneal incision
is enlarged in order to place the IOL
- pars plana phaco: the decreased total US energy avoid scleral burst
risk
