HUNDRED SIXTY DEGREES RETINOTOMY IN SEVERE PVR

Purpose: To report
the anatomical and functional results of extensive (360-degree) retinotomy
in eyes with retinal detachment (RD) complicated by severe proliferative
vitreoretinopathy (PVR).
Method: This series includes 44 eyes with RD complicated by very severe
PVR where epiretinal and/or subretinal membrane removal together with
less extensive retinotomy has failed to achiever retinal reattachment
due to extensive anterior PVR and/or intrinsic retinal shortening. Three
hundred and sixty degree retinotomy was then performed before condemning
these eyes as inoperable.
Results: Succesful intraoperative retinal reattachment was achieved in
36 eyes (81.8 %). Eyes with attached retina were followed up for a period
between 12 and 55 months ( mean 26 months). Redetachment occurred in 5
eyes due to recurrent PVR . All eyes with attached retina had some degree
of visual improvement
Conclusion: 360-degree retinotomy is helpful in achieving retinal reattachment
and visual improvement in some eyes with severe PVR which could not be
treated by other simpler methods.
