SLIT
LAMP AND FLOW CONTROL ADVANTAGES FOR PVR SURGERY
Didier Ducournau, MD
Nantes, France
The use of the slit
lamp and vitrectomy machine allowing the aspiration flow control decrease
some of the vitrectomy complications in the treatment of retinal detachment:
- The post operative PVR produced by leaving anterior vitreous and posterior
hyaloid will be decreased by
the machine: the aspiration flow control will allow to remove more vitreous;
the peristaltic pump will also allow an easier removal of the posterior
hyaloid;
the slit lamp.will allow the continuous observation of the retina, from
the posterior pole to the ora serrata, while performing the manual peripheral
indentation, which will be done by the surgeon.
- The risk of iatrogenic tears, often due to the lack of control of the
aspiration flow will be decreased.
- The cristallin lens touch risk will be decreased with the help of the
optical cut provided by the slit lamp. No touch by sceral fixed illumination
during peripheral indentation is to fear.
-The vitreo retinal incarceration risk will be decreased by: the slit
lamp
which allows to perform a 2 port vitrectomy
which allows to perform a complete vitrectomy without exit. In fact, with
an intraocular fiber, if you want to make the peripheral indentation by
yourself you have to take out the fiber and then decrease the incarceration
risk
