PVR
AND RD IN THE ELDERLY

Purpose: To evaluate
incidence and influence of proliferative vitreoretinopathy(PVR) associated
with retinal detachment(RD) in the elderly, and other prognostic factors.
Patients and method : Follow-up from January 1998 to December 2002 of
52 caucasian patients aged of 80 years or more (mean age of 87 range 80
to 94 years) undergoing surgical procedures (37 with scleral buckle and
15 with pars plana vitrectomy) for retinal detachment. Characteristics
of RD, PVR, anterior ocular pathology(pseudophakia, trauma, myopia) and
clinical results are analysed.
Results : Mean follow-up was 2.7 years (range 6 months to 5 years) : retina
was detached in 3, 18, 15 and 6 cases respectively in 1, 2, 3 and 4 quadrants,
macula detached in 38 cases. There was preoperative PVR in 29 cases(56%)
: 7 stade A, 18 stade B and 4 stade 4. 3 eyes presented postoperative
PVR and second surgical procedure was necessary in 5 cases.. Time of ocular
findings before surgery, status of macula and importance of PVR appeared
statisticaly as the main prognostic factors for success of surgery and
clinical results.
Conclusion : In our series, there is a high incidence of PVR associated
with retinal detachment in the elderly. The level of PVR influences the
kind of surgical procedure and the final clinical results.
